There is limited knowledge about cancer patients' experiences of uncertainty while waiting for genome sequencing results, and whether prolonged uncertainty contributes to psychological factors in this context. To investigate uncertainty in patients with a cancer of likely hereditary origin while waiting for genome sequencing results, we collected questionnaire and interview data at baseline, and at three and 12 months follow up. Participants had negative attitudes towards uncertainty at baseline, and low levels of uncertainty at three and 12 months. Uncertainty about (...) genome sequencing did not change significantly over time [t = 0.660, p = 0.510]. Greater perceived susceptibility for cancer [r = 0.14, p < 0.01], fear of cancer recurrence [r = 0.19, p < 0.01], perceived importance of genome sequencing [r = 0.24, p < 0.01], intention to change behavior if a gene variant indicating risk is found [r = 0.29, p < 0.01], perceived ability to cope with results [r = 0.36, p < 0.01], and satisfaction with decision to have genome sequencing [r = 0.52, p < 0.01] were significantly correlated with negative attitudes towards uncertainty at baseline. Multiple primary cancer diagnoses [B = −2.364 [−4.238, −0.491], p = 0.014], lower perceived ability to cope with results [B = −0.1.881 [−3.403, −0.359], p = 0.016] at baseline, greater anxiety about genome sequencing [B = 0.347 [0.148, 0.546], p = 0.0012] at 3 months, and greater perceived uncertainty about genome sequencing [B = 0.494 [0.267, 0.721] p = 0.000] at 3 months significantly predicted greater perceived uncertainty about genome sequencing at 12 months. Greater perceived uncertainty about genome sequencing at 3 months significantly predicted greater anxiety about genome sequencing at 12 months [B = 0.291 [0.072, 0.509], p = 0.009]. Semi-structured interviews revealed that while participants were motivated to pursue genome sequencing as a strategy to reduce their illness and risk uncertainty, genome sequencing generated additional practical, scientific and personal uncertainties. Some uncertainties were consistently discussed over the 12 months, while others emerged over time. Similarly, some uncertainty coping strategies were consistent over time, while others emerged while patients waited for their genome sequencing results. This study demonstrates the complexity of uncertainty generated by genome sequencing for cancer patients and provides further support for the inter-relationship between uncertainty and anxiety. Helping patients manage their uncertainty may ameliorate psychological morbidity. (shrink)

Sartre posed a problem for himself in his discussion of bad faith: how is it possible to deceive oneself, given the unity and translucency of consciousness? Many critics of Sartre interpret translucency as transparency; some, such as M.R. Haight, conclude that Sartre's account of consciousness makes self-deception impossible.A reply to those critics takes the form of showing that translucent consciousness has a number of dimensions: (a) non-positional versus positional aspects; (b) prereflective versus reflective levels; (c) temporally synthetic flux; and (d) (...) the first-person perspective versus the third-person perspective. These dimensions enable Sartre to succeed in describing subtle and varied patterns of self-deception, based on such strategies as obscuring, evasion, distraction, misdescription and disavowal. The translucency of consciousness is not a barrier to self-deception.However, there is another problem in Sartre's claim that all purposive activity is conscious, including our practice of self-deception. The bodily subject of consciousness performs important purposive activities beyond the range of the most obscure non-positional consciousness. This calls into question Sartre's existentialist claim that we are wholly responsible for all we do. (shrink)

In the last decades, Systems Biology (including cancer research) has been driven by technology, statistical modelling and bioinformatics. In this paper we try to bring biological and philosophical thinking back. We thus aim at making diferent traditions of thought compatible: (a) causality in epidemiology and in philosophical theorizing—notably, the “sufcient-component-cause framework” and the “mark transmission” approach; (b) new acquisitions about disease pathogenesis, e.g. the “branched model” in cancer, and the role of biomarkers in this process; (c) the burgeoning of omics (...) research, with a large number of “signals” and of associations that need to be interpreted. In the paper we summarize frst the current views on carcinogenesis, and then explore the relevance of current philosophical interpretations of “cancer causes”. We try to ofer a unifying framework to incorporate biomarkers and omic data into causal models, referring to a position called “evidential pluralism”. According to this view, causal reasoning is based on both “evidence of diference-making” (e.g. associations) and on “evidence of underlying biological mechanisms”. We conceptualize the way scientists detect and trace signals in terms of information transmission, which is a generalization of the mark transmission theory developed by philosopher Wesley Salmon. Our approach is capable of helping us conceptualize how heterogeneous factors such as micro and macro-biological and psycho-social—are causally linked. This is important not only to understand cancer etiology, but also to design public health policies that target the right causal factors at the macro-level. (shrink)

The thesis that there can be vague objects is the thesis that there can be identity statements which are indeterminate in truth-value (i.e., neither true nor false) as a result of vagueness (as opposed, e.g., to reference-failure), "the singular terms of which do not have their references fixed by vague descriptive means". (if this is "not" what is meant by the thesis that there can be vague objects, it is not clear what "is" meant by it.) the possibility of vague (...) objects should not be taken, in itself, to imply the more radical thesis that the identity relation can be one of "degree". one can hold that the concept of degrees of identity is absurd (how can one thing be more or less identical to another?) "and" that indeterminacy in identity is possible; hence, any incoherence in the idea of degrees of identity does not thereby undermine the idea of indeterminate identity. (shrink)

This article presents a brief general view of the recent literature and the scholarly activity in the field of philosophy of medicine in Scandinavia. The focus of attention is not on medical ethics, but on studies on topics like decision theory, medical classification, causality, causal explanations, concept formation, and on analyses of different ideals of medical science and clinical practice. A few principal works on medical ethics are mentioned by way of introduction and a brief account of a highly topical (...) debate on the legislation on artificial insemination in Sweden is given at the end. This paper considers two of the most prominent kinds of evidence that have been used to argue that certain embedded questions receive weakly exhaustive interpretations. The first kind is exemplified by judgments of consistency for declarative sentences that attribute knowledge of a wh-question and ignorance of the negation of that question to the same person, and the second concerns asymmetries between the role of positive and negative information in validating question-embedding surprise ascriptions, and similar judgments for other attitudes. I argue (...) that neither type suffices to show weak exhaustivity. The first can be analyzed in terms of strong exhaustivity in combination with domain restriction effects, while the second can be analyzed in terms of a mention-some interpretation. These kinds of evidence have served as the empirical basis for many claims about weakly exhaustive readings, so the observation that they are unreliable calls into questions a large body of established work on the semantics of question embedding. The International Institute for the Study of Man has promoted a research theme charged with a project of reforestation of the Atlas Mountains to be proposed to the E.C.The Atlas Project relies on three fundamental assumptions: a. there is the need to build CO2 sinks that, at the same time, are a source of energy and income in regions from which, due to the lack of both, vast migratory flows start. The state members of the European Community are not able (...) to absorb the quantity of CO2 produced by industrial combustion, the heating system and cars, and they still do not follow the international agreement on pollution control;b. a regular stream of air from Northern Europe discharges its humidity on the Atlas Mountains, before it re-ascends over Europe. This stream of air could be a natural carrier for conveying and immobilising the CO2 produced in the industrial regions of Europe through its fixation in timber on the Atlas Mountains before it re-ascends over the European continent;c. the Moroccan area, and North Africa in general, are the source of a large number of immigrants into Europe due to the high population pressure and the lack of employment. In his account of probable reasoning, Poincaré used the concept, or at least the language, of conventions. In particular, he claimed that the prior probabilities essential for inverse probable reasoning are determined conventionally. This paper investigates, in the light of Poincaré's well known claim about the conventionality of metric geometry, what this could mean, and how it is related to other views about the determination of prior probabilities. Particular attention is paid to the similarities and differences between Poincaré's conventionalism as (...) it applies to probabilities and de Finetti's subjectivism. The aim of the paper is to suggest that in accounts of the development of ideas about probable reasoning, particularly those customarily described as Bayesian, Poincaré's discussion deserves more attention than it has so far received.
